What is the best way to reintroducie food to my body? Is there diet plan anyone has used that I can have?

Yep, Saria's right, knowing whether you're fasting on water or juice is essential. If you've been fasting on water, for sure start with some gentle fresh juices. I like to break water fasts with freshly juiced apples.

Speltstrong, I am planning on having freshly juiced apples for breaking the water fast I'm on!! I have them in the fridge already!

I broke my month long juice fast by having a small bowl of grapes for dinner the first day, after having continued on juice all that day. The second day I had some melon along with continuing on juice. I think it was the third day I had a small salad of lettuce, mashed avocado and lemon juice. Hahahahaha -- it was the BEST salad I ever had!!
By the fourth day I was fine to start reintroducing more vegetables. Just use common sense and be gentle with yourself, you'll be fine.

Qwerty has a good plan...Just listen to your body as you go. If anything really bothers you wait another 24 hours before having more solids. Its best to have fruits first, then move on to vegetables.

Stick to raw food for as many days that you fasted. Chew your food, until it is become like water in your mouth. I would start with solid fruits first, then vegetables and other raw food. I wouldn't eat more then two meal a day and no more then the quantity of 2 kilogram each time, ideally 1 kilogram. You don't want to over taxes all of your digestive system
Congratulation on your fast, this is wonderful, you must feel so great.
I would be a perfect time to do a 7 days water fast, just something to consider.
