Doesn't include many items as it's geared towards those with illnesses like cancer, but gives you an idea of how to make your own.

I was wanting to make one of these. This helps with a way to word things.
Thanks!
it should specify no sauces or seasoning of any kind as well. i was served a plate of lovely raw mango slices... drenched in sugar syrup. eww.

This reminds me of the 'vegan passport' a little booklet that has instructions on what vegans eat in about 50 different languages. And if language doesn;t work, it has a picture thingy, with a smiley face above the vegan food and a frowny face above the non vegan food.

coco, I get that ALL the time! I sit there having a conversation about the fact that it doesn't have to have anything on it, and they can't even grasp the idea, not even for a plate of fruit!
I went to an asian restaurant about 6 months ago (yes, this was bound to be difficult I know, but it was a good friend and I didn't want to miss it). I had a back and forth conversation with the owner about bringing out a plate of chopped veggies. Just. Chopped. Veggies.
He at first refused citing health reasons (???) but with my humour and charm I can get people to almost anything and convinced him that I wouldn't die, even though the vegetables weren't organic... ;) He was so intrigued at the end.

I always make sure to specify plain with nothing on it and fruit, will definitely say no sugar or sweetener. I always make sure people know that I will get sick if the food is cooked or has anything added to it. Not that I eat out much anyhow. I'm usually very prepared by checking out their website/menu first. Eating at home is way more fun usually, but not always an option. Unless of course it's a nice raw place.
no matter what you say there are more people working in kitchens and dining rooms who will make/bring you your plain salad or fruit plate or whatever with something extra (inedible) on it as a nice little freebee to dress it up for you. seriously, how many times and ways can i say just plain with nothing at all on it? it just doesn't compute most of the time. they can't believe that you would want that to eat, it seems like a test to them and they are determined not to fail. it's freaky.
this was way more of an issue before, now i am not so stringent with my diet so i can eat in restaurants without this hassle.

Thanks GlimR, you've given me more ammo to get that salad/fruit just right!
