I take Jarrow's. My naturopath used to sell it and I trust the company. I only take it once in a blue moon when I feel a little off and figure, my stores of B-12 must be low. It's available at health food stores and vitamin stores such as Super Supplements (West Coast?).
My holistic MD also recommends Jarrow (http://www.iherb.com/Jarrow-Formulas-Methyl-B-12-5000-mcg-60-Lozenges/117?at=0) - and that's what my husband and I both take. I order it through iHerb. We each take 5000mcg daily. *Ü*

5000 seems like a lot. What's the reason for such a high dose?

You don't need more than 5000 mcg a week, probably even less. B-12 accumulates in your body, so you don't really get deficient until you have not taken any in several weeks, even months. I don't think there is any benefit to storing that large an amount in your body.

I take 2 under the tongue every week or every other week.
The reason the dosages are so high is because it's a sensitive little thing, and a lot of that isn't going to be going into the body - it's not viable in the pill anymore, then a lot of what goes into the body isn't uptaken. If indeed uptaken is a word!:p
In fact I've read that methylcobalimin is fairly unstable, and that's why so many companies go with the cyannocobalimin, because it is more stable and more "guaranteed" that you will actually get some of it. but then you the cyanide left over.
