Alissa's Mayo

I am in the midst of moving - am moving on Saturday. Last night I made the mistake of packing my un-cookbooks, even though I'd bought the ingredients for Alissa's Mayo, to make coleslaw. Now I can't find the box, or remember what portions....
If anyone can help it would be greatly appreciated!!!
